A patient referral for a procedure can also be submitted by printing a referral form here and faxing the completed form with order, clinic notes, MRI, other imaging and patient demographic information to 763.767.7193.
Once we receive the form, a Nura patient care coordinator will verify insurance, procure authorization immediately and then contact the patient to schedule the procedure. After the patient is seen, we will communicate the procedure note back to you.
